In 1999, a report was released titled To Err is Human, which estimated that between 44,000 and 98,000 Americans died in 1997 as a result of preventable medical errors (Institute of Medicine, 1999).

  • Discuss why preventable medical errors are still a problem in the United States almost 20 years later.
  • Furthermore, describe voluntary and involuntary reporting, the advantages and limitations of each, and which method may do more to reduce preventable medical errors.
